Add or remove fields from your form
You have the flexibility to include certain fields in your projects' forms.
Add fields to your form
Once you select a work type for your form, the Fields section will display all the fields that are available for that work type. Simply drag and drop the field you’d like to include in your form, and then edit them.
Remove fields from your form
To remove a field from your form:
Select a field.
Select Remove field ().
Or alternatively, you can drag the field back over to the Fields section.

The following system fields are supported in forms:

Attachment
Due date
Priority
Labels
Custom fields can also be added. These include:
Assignee
Affected services (software projects)
Category
Components (software projects)

People (team-managed projects)
Radio buttons

You may want to include more details in your forms beyond the default fields. A project admin can do this by setting up a new work type scheme and then adding custom fields. More about custom fields
Restricted fields in public forms
Forms set to public access have some fields that are restricted because they may contain sensitive or confidential data. You won’t be able to add a restricted field to a form with public access, and if you’ve already added a restricted field, it won’t be shown to anyone who views or responds to the form.
Restricted fields include:
Assignee
Attachment
People
Parent
Team
Sprint
Project
Labels
Fix versions
